Fall Creek Valley Care Center is a 60 bed skilled care facility featuring both private and
semi-private rooms, three dining areas, lounges, family room and a chapel. The original building was built in 1968 with
additions in 1974, 1983, and 1989. The facility has been well-maintained and updated.
We are approved and contracted with the U.S. Department of Health and Human Services for Medicare and Medical
Assistance recipients. Additionally, we participate in several area health systems, private insurance companies, and
have contracts with area Home Care and Hospice providers. Private pay residents also select us frequently because of
our excellent meals, quiet and pleasant surroundings, and conscientious attention to their needs.
